Why this directory exists

Finding an appropriate provider can be difficult when cost, location, language, transportation, or uncertainty stands in the way. Illuminate Community Connect brings provider information and community support together so people can explore a clear next step.

Illuminate Foundation’s contribution

This website was donated by Illuminate Foundation to help address barriers to finding an appropriate provider. The directory welcomes everyone. Public access and eligible provider listings are free; services listed may have their own fees and eligibility requirements.

Independent providers, equal visibility

Listed providers and organizations operate independently. Inclusion is not an endorsement, recommendation, clinical assessment, or guarantee. Listings follow the same format and search rules, with no paid priority placement. Resources must serve Garfield or Pitkin residents directly, regionally, or through appropriately available online services. Clinical care, peer support, and community resources are identified separately where the source supports the distinction.

What has—and hasn’t—been confirmed

The initial directory was imported from a community spreadsheet whose original update dates vary. Imported entries show “Last confirmed: not confirmed.” No provider-confirmation dates, dated openings, or license-check dates have been invented. Group-practice information does not automatically apply to each clinician.

For clinical listings, the review process should include checking applicable credentials through Colorado’s official verification service and recording the date. A license check confirms only that specific check, not treatment quality or fit.

How requests are reviewed

New listings, updates, corrections, and removals go through administrative review. The team checks relevance and completeness and confirms details with the provider or organization when needed. Community suggestions are research leads, not provider-authorized submissions. Only approved public business information is published.

Keeping the directory useful

The maintenance standard is to request core-detail confirmation about every 90 days and check openings more often. Availability should be dated when reported, and older or undated status shown as “Contact provider for current availability.” This is the intended review standard; automated reminders and a recurring review schedule have not been activated.

Administrative ownership is through the directory team designated by Illuminate Foundation. A named administrator and any dedicated directory inbox should be assigned before public launch. The form currently uses the existing Squarespace account notification destination; notifications are disabled during the trial.
